CMS/Ped-I-Care is a Provider Service Network (PSN) serving children who qualify for coverage under Title XIX in Duval County as part of Medicaid Reform.  The University of Florida, Department of Pediatrics, Integrated Care System (ICS/Ped-I-Care) has contracted with the Department of Health/Children’s Medical Services Network (CMSN) to form this PSN to serve children with special health care needs and their siblings up to the age of 21, effective May 1, 2007.   This PSN is implemented in collaboration with the Jacksonville CMS office, which continues to provide nurse care coordination for the members and providers.  CMS/Ped-I-Care members continue to access pharmacy services through their Medicaid benefits.

All CMS/Ped-I-Care Title XIX Medicaid Reform PSN participating providers are advised to enroll in the Vaccines For Children (VFC) program and administer these immunizations to their eligible CMS/Ped-I-Care Title XIX members. You may request an enrollment packet by faxing a request to the VFC program at 850-245-4734 and providing the following information:

Your practice name and mailing address

Contact person, phone and fax numbers, and email address
