Ped-I-Care members do not apply directly to Ped-I-Care when they join. To join Ped-I-Care, you must apply to Florida KidCare and your child must be accepted by CMS with a special health care need.

Either way, you'll need the following information when you apply:

◊ Your family's most recent tax return (Form 1040
) OR -
◊ Your Wage and Earning Statement (W-2 Form
) OR -
◊ Current Pay Stubs (covering the last 4 weeks
) -
◊ The cost to add your child or children to health insurance coverage if your employer offers it.

◊ Your children's Social Security numbers or the date applied for if you have not received a Social Security card.
