Your CMS Nurse Care Coordinator is always available to discuss health care concerns. They will assist you and your PCP with managing and coordinating your child’s care. Your Nurse Care Coordinator will assist you in arranging the following services:
- Transportation
- Dental Care
- Nutrition Services
- Prescription Drugs
They can also assist you and your PCP with referrals to specialists, therapy and other providers.
