- Provider "Credentialing"
Through our partnership with Children’s Medical Services (CMS), Ped-I-Care serves children with special health care needs. All Ped-I-Care providers must be "credentialed"/approved by CMS to provide services to CMS/Ped-I-Care members.
New providers to the Ped-I-Care network who are not already CMS approved will need to complete an online CMS Approval Application. If you require support with this application process, please email: cmsproviderhelp@doh.state.fl.us or call 1-866-376-2456.

Once approved, physician providers must be re-approved every three years.
